Local Service Request Help

Use these instructions to help you enter information into the Local Service Request form. You may also access the entire AT&T Business Services Local Number Portability Procedures Handbook from the Local Number Portability for Business web page.

Note: Fields whose names are boldfaced are required fields.

Administrative Section

Field Description and Instructions
Type of Service (Service Center) Select Service Center to receive this form. 7125 is for AT&T LNS Local Service requests. 7421 is for AT&T Digital Link requests.
PON Purchase Order Number: Identifies the customer's unique purchase-order or requisition number that authorizes the issuance of this request or supplement : Required
ATN Account Telephone Number: Identifies the account telephone number assigned by the NSP. Source of this would be the BTN on the old LSP's CSR : Required
CCNA Customer Carrier Name Abbreviation: Identifies the COMMON LANGUAGE Interconnection Access Code (IAC) for the customer submitting the LSR and receiving the response. This field will contain the ACNA code of the new LSP : Required
CC Company Code: Identifies the Exchange Carrier requesting service. Contains the SPID of the new LSP : Required
NNSP New Network Service Provider Identification: Identifies the Number Portability Administration Center (NPAC) Service Provider Identifier (SPI) of the new Network Service Provider: Conditional; On porting orders, must contain the SPID of the new NSP when it is different from that provided in the CC field
LSP Name Local Service Provider Name: Submitting provider's name : Required
NPDI Number Portability Direction Indicator: Required field: C = Wireline to wireless, D = Wireline to wireline
Supplemental Request? Select yes if this is a new iteration of a LSR : Optional
SUP Supplement Type: Identifies the reason for which the supplement is being issued. Valid entries: 1=Cancel, 2=New Desired Due Date only, 3=Other. When entry of "3" is used, enter the reason in the REMARKS field : Conditional; field becomes Required if Supplemental Request box is checked.
VER Version Identification: Identifies the customer's version number of the LSR : Conditional; unique value required if Supplemental Request is checked. Valid entries may be AA-ZZ or 00-99.
EXP Expedite: Indicates that expedited treatment is requested and any charges generated in provisioning this request (e.g. additional engineering charges or labor charges if applicable) will be accepted : For SC 7125, any submission that is under 4 days, with a 3PM Mountain Time cutoff, must be expedited. (EXP=Y)
EXP REMARKS Expedite Remarks: List a short reason for expediting the request : Required if EXP=Y
DDD Desired Due Date: Identifies the customer's desired due date: Required
CHC Coordinated Hot Cut: Identifies that the customer is requesting near seamless cutover activity : Optional.
DDD-Time Desired Due Date - Time: Identifies the hour and minute customer's desired due date: Optional.
DFDT Desired Frame Due Time: Identifies desired frame cutover time. This is only supported by AT&T Digital Link, Service Center 7421
REQTYP Requisition Type: Identifies the type of service being requested and the status of the request : Required non-editable field : C=Number Portability (Port TN only) B=Firm Order
ACT Activity: Identifies the activity involved in this service request : Required non-editable field : V=Conversion to new LSP
MI Migration Indicator: Identifies the type of account level activity when lines/numbers are converting from one LSP to another LSP : Required; valid entries are A=Partial migration, C=Full migration
AGAUTH Agency Authorization Status: Indicates that the customer is acting as an end user's agent and has authorization on file : Required; box must be checked
DATED Date of Agency Authorization: Identifies the date appearing on the agency authorization that was previously submitted to the provider : Required; supplied for end user customer care purposes to validate the date the authorization for migration was supplied.
AUTHNM Authorization Name: Identifies the end user who signed the authorization : Required; supplied for end user customer care purposes to validate who supplied authorization for migration.
RPON Related Purchase Order Number: Identifies the PON of a related service request : Optional
RORD Related Order Number: Identifies the related provider order number : Optional

INIT Initiator Identification: Identifies the customer's representative who originated this request : Required
TEL NO Telephone Number: Identifies the telephone number of the initiator: Required
EMAIL EMAIL Address: Identifies the electronic mail address of the initiator: Optional
FAX NO Facsimile Number: Identifies the fax number of the initiator : Optional

Location and Access

NAME End User Name: Identifies the name of the end user : Required
AFT Address Format Type: Identifies the format of the address being supplied. This instance of the AFT field is used for the service address: Optional; valid entries: A = Rural Route and or Box Number, C = Provider Assigned House Number, D = Descriptive, E = Provisioning Address
SAPR Service Address House Prefix: Identifies the prefix for the house number of the service address when grid type numbering is used : Conditional; populate if applicable, and not supplied in the SASN field as a part of a full street address.
SANO Service Address House Number: Identifies the house number of the service address : Conditional; populate if applicable, and not supplied in the SASN field as a part of a full street address.
SASF Service Address House Number Suffix: Identifies the suffix for the house number of the service address: Conditional; populate if applicable, and not supplied in the SASN field as a part of a full street address.
SASD Service Address Street Directional: Identifies the street directional for the service address : Conditional; populate if applicable, and not supplied in the SASN field as a part of a full street address.
SASN Service Address Street Name: Identifies the street name of the service address : Required
SATH Service Address Thoroughfare: Identifies the thoroughfare portion of the street name of the service address : Conditional; populate if applicable, and not supplied in the SASN field as a part of a full street address.
SASS Service Address Street Suffix: Identifies the suffix to the street name of the service address: Conditional; populate if applicable, and not supplied in the SASN field as a part of a full street address.
LD1 Location Designator 1: Identifies additional specific information related to the address (e. building, floor, room). This instance of the LD1 field is used for the service address : Optional
LV1 Location Value 1: Identifies the value associated with the first location designator of the address. This instance of the LV1 field is used for the service address : Optional when the LD1 field is populated, otherwise prohibited.
LD2 Location Designator 2: Identifies additional specific information related to the address (e. building, floor, room). This instance of the LD1 field is used for the service address : Optional
LV2 Location Value 2: Identifies the value associated with the first location designator of the address. This instance of the LV2 field is used for the service address : Optional when the LD2 field is populated, otherwise prohibited.
LD3 Location Designator 3: Identifies additional specific information related to the address (e. building, floor, room). This instance of the LD3 field is used for the service address : Optional
LV3 Location Value 3: Identifies the value associated with the first location designator of the address. This instance of the LV3 field is used for the service address : Optional when the LD3 field is populated, otherwise prohibited.
CITY City: Identifies the city, village, township, etc. This instance of the CITY field is used for the service address : Required
STATE State: Identifies the State. This instance of the STATE field is used for the service address : Required
ZIP ZIP/Postal Code: Identifies the Zip Code, ZIP code + extension or postal code. This instance of the ZIP field is used for the service address : Required
LCON Local Contact: Identifies the local contact name for access to the service location : Optional
TEL NO Telephone Number: Identifies the telephone number of the local contact : Optional
EUMI End User Moving Indicator: Indicates that the end user location is changing : Optional: Values are N=End user is not moving (default) and Y=End user is moving

NPQTY Number Portability Quantity: Identifies the quantity of ported numbers involved in this service : Required
LNA Line Activity: Identifies the activity involved at the line level : Non-editable field; valid entry: V = Convert to new LSP
TDT Ten Digit Trigger: Indicates the request for the activation of a ten-digit trigger for local routing number portability : Required when the NPT field is populated with "D". Valid Entry: Y = Yes : Optional for SC = 7125 AT&T LNS Local Services Prohibited for SC = 7421 AT&T Digital Link
PORTED NBR - RANGE Ported Telephone Number: Identifies the telephone number to be ported. This field accommodates either single 10 digit TN or range of TNs to be ported : Required.
NPT Number Portability Type: Indicates the type of number portability for this request : Required non-editable field. Only applicable valid entry is: D = Local Routing Number
