AT&T participates in the Rural Health Care Program. Rural Health Care applicants may contact AT&T at 877-313-2546. Health Care Providers in need of information about the Rural Health Care Program may contact the Rural Health Care Division on 800-229-5476. What is a SPIN and why is it important?
SPIN stands for Service Provider Identification Number. The SLD keeps track of the hundreds of vendors providing services to schools and libraries around the country by assigning each vendor a unique SPIN based on their Form 498 application to participate in the program. The Form 498 asks for information such as Federal Registration Number for the company, etc… To continue to participate in the E-rate Program, Service Providers must file a Service Provider Annual Certification (SPAC) - Form 473 certifying to program rules. Only Service Providers who provide services on a common carrier basis are eligible to provide services for Telecommunications Service category.
The AT&T family of companies offers many E-rate eligible services in the categories of Telecommunications, Internet Access, and Internal Connections as well as the Basic Maintenance of those Internal Connections. It is important that you use the appropriate SPIN associated with the products offered for the AT&T Company proving a particular service. If you are uncertain which SPIN is appropriate please contact your account executive or visit AT&T Service Provider Information Numbers (SPINs) for contact information.
What services does AT&T offer that qualify for E-Rate ?
E-rate funding is available for four categories of Service in two priorities, and the AT&T family of companies can provide services in all of them. Please speak to your AT&T Account Executive, or if you do not have an Account Executive contact the number on your bill to discuss AT&T products & services.
Disclosure: AT&T can discuss our products and services prior to the posting of your Form 470, which begins the competitive bidding process. Once the Form 470 is filed AT&T is restricted to the role of bidder.

Once funded on an AT&T SPIN, what can I expect?
You will receive an AT&T welcome notice and instructions from the AT&T National E-rate Center of Excellence for each AT&T SPIN you identified on your Form 471 application(s) for each Funding Request.

E-Rate disbursements methods are:
a) Billed Entity Applicant Reimbursement (BEAR) - Form 472
b) Service Provider Invoice (SPI) - Form 474

The SLD recommends filing BEAR Form 472’s online. AT&T participates in the SLD on-line BEAR process for the FCC Form 472.

ARF (AT&T Applicant Reimbursement Form) What is it?
ARF stands for AT&T Reimbursement Form. It is the method for utilizing the SPI discount method only available for use with the following SPINs:
- 143001192 (AT&T Corp.)
- 143005617 (AT&T Alascom)
- 143000067 (Teleport Communications Group, Inc.)
- 143001113 (AT&T Corp ACC Long Distance Corp)
ARF is AT&T's online method for requesting E-Rate discounts for the SPI method of receiving Funding. For these AT&T SPINs there are no discounts applied directly to the bill if the SPI method is chosen. To use the ARF online web site you will be issued a PIN. Upon receipt of an E-Rate Funding Commitment Decision Letter, AT&T sends a "PIN" letter to E-Rate participants who are not current users of the ARF web site. AT&T must also be in receipt of your FCC Form 486 notification letter, sent to both the Service Provider and the participant by Schools and Libraries Division, before an E-Rate reimbursement can be processed.
